Fifth Grade Memory Books

Step 1: Rock Star Profile
(complete by November 20, 2009)
Please have your child complete the questionnaire about themselves, their favorite things, and their memories of HCE to include in the Fifth Grade Memory Book.

Cookies and pop-ups must be enabled to use all the features available in the survey.

You can also have your child complete the profile online in the school library before or after, but not during, school hours.

If your child would like to think about their responses and make notes, click the link at right to print the questions. You can submit your responses until November 20th.

Step 2: Rock Star Head Shot and Photographs
(submit by November 20, 2009)
Rock Star Photos for the Memory Book and graduation video will include:

• One baby picture (six months or older) - Provided by you
  
  
• Five to seven other photos (photos with friends, family vacations, participating in activities, HCE events, etc.) - Provided by you
  
  
• "Rock Star Head Shot" - Provided by HCE PTA

There are three options for submitting the pictures (in order of preference):

1. Email your digital photos or scans as attachments to memorybooks@hcepta.com and include your child's first and last name as the Subject Line.
   Because the Memory Books will be printed, make sure the resolution of your scans or files is 300 DPI to ensure they print properly.
   
   
2. Work with the Memory Book Committee to scan and submit them at the school library between 11:30-12:30 on Tuesdays and Thursdays.
   
   
3. Include them in the envelope provided with the Fifth Grade Graduation packet. They will be returned to you in early December.
